ATTENTION: By signing this document, Vendor fully agrees to the below conditions without exception.

Terms of Use

Vendor will issue a house account to identify this House Account pursuant to the instructions above. If payment by check, then Vendor or shiva.com and/or eCondolence will send a statement of House Account charges to the address and/or email indicated above at the end of each month. shiva.com and/or eCondolence acknowledges responsibility to pay the total undisputed amount of all purchases and delivery fees accurately reflected on the House Account Statement.

Acceptance of Terms of Use

Vendor shall fulfill orders received by shiva.com and/or eCondolence via the delivery arrangement. Each order received on or before Noon shall be fulfilled the same day based upon the agreed upon shipping/delivery schedule. Vendor shall include shiva.com’s and/or eCondolence’s label, logo and collateral, if any provided by shiva.com together with any other promotional materials and content to be used for fulfillment of the products. Vendor shall include shiva.com label on packaging for each order fulfilled. Vendor shall display shiva.com decal at establishment. Shiva.com is the sole owner of any information that shiva.com collects from users/customers for delivery, and Vendor shall hold all customer data in strict confidence. Further, Vendor agrees to comply with the Vendor Code of Conduct located at www.shiva.com/code-of-conduct/.

shiva.com and/or eCondolence and Vendor may agree upon what products/services and delivery fees will be incurred. Either party may cancel this agreement with thirty days’ notice at any time for any reason by giving written notice to the other.
